YOUR TRANSITION JOURNEY WITH ZENITH LIVING SERVICES
STEP 1
OPTIONS COUNSELING AGENCY
The Medicaid Transition Services Options Counseling program is designed to provide guidance for Medicaid members residing in skilled nursing facilities, regional centers, and intermediate care facilities. These agencies are dedicated to helping long-term residents explore the possibilities of transitioning back to community living.
At Zenith Living Services, our Transition Services team will work closely with you to select an options counseling agency and assist you in getting enrolled to initiate your transition journey.
STEP 2
MEETING WITH YOUR TRANSITION TEAM
Upon completing your Options Counseling referral form, our Transition Care Advocate (TCA) will reach out to you to discuss the next steps for determining your eligibility and crafting your personalized transition plan.
STEP 3
INTAKE ASSESSMENT
Our dedicated team will conduct an intake assessment, collaborating with you to gather essential information crucial for a seamless transition into the community.
STEP 4
YOUR HOUSING VOUCHER AND HOUSING SEARCH
Our transition services team will liaise with the Department of Housing to assess your eligibility for their housing navigation services. This evaluation will consider your financial status and any past eviction history.
To move forward, you'll be required to attend a housing voucher informative session. Upon completion, you will receive a housing voucher to initiate your search for suitable housing with rent support.
STEP 5
MOVING IN
Medicaid provides a modest stipend to the Transition Agency, enabling us to purchase furniture, household essentials, and groceries essential for a successful living situation.
Our team will assist in gathering furniture and facilitating its move into your new apartment, ensuring a smooth transition as you embark on this exciting new chapter in the community.
STEP 6
LIVING INDEPENDENTLY WITH COMMUNITY SUPPORT
At Zenith Living Services, we collaborate with several Home Health Agencies to facilitate the completion of your HCBS waiver services. Our commitment is to help you find a caregiver to ensure you receive the vital in-home support necessary for your continued success in independent living within the community.
